Introduction and Objective: BMI is the traditional indicator for obesity diagnosis, but it has limits due to ignoring fat distribution. Abdominal visceral fat (VF) is a key risk factor of metabolic diseases. This study aims to explore the relationship between visceral fat and obesity-related metabolic disease and further validate EASO criteria in a Chinese population, propose a new standards for identifying visceral fat using simple anthropometric indicators. Methods: A cross-sectional study of 3,371 participants aged 26-76 in Pinggu District, Beijing were assessed anthropometric measurements, metabolic indicators, and visceral fat areas (VFA) via CT scans. We used the McNemar test to compare the diagnostic accuracy of the EASO criteria and BMI for identifying visceral obesity. The correlation between anthropometric indicators and VFA was analyzed using Spearman's correlation. ROC curves were used to compare the diagnostic efficacy of anthropometric indicators with EASO criteria. Results: Among 3371 participants, 61.2% were diagnosed as visceral obesity. Metabolic diseases, including pre-diabetes, DM, hypertension, hyperlipidemia, and fatty liver, were more prevalent in those with visceral obesity, suggesting visceral fat as a sensitive indicator for identifying metabolic diseases. The EASO criteria showed higher sensitivity (71.7% vs 21.5%) and lower specificity (84.6% vs 98.5%) compared to BMI alone for diagnosing visceral obesity. WC and WHtR correlated strongly with VFA. BMI combined with WC was the most effective tool for diagnosing visceral obesity, with optimal cutoffs of BMI ≥ 24.82 kg/m² and WC ≥ 87.65 cm for men, and BMI ≥ 25.26 kg/m² and WC ≥ 84.75 cm for women. Conclusion: Visceral fat is associated with metabolic diseases, EASO criteria is superior to BMI for identifying visceral obesity.
