Classificatory treatment of refractory idiopathic thrombocytopenic purpura

Objective To explore the effective treatment of refractory idiopathic thrombocytopenic purpura(ITP).Methods Forty-one adult patients with refractory ITP were analyzed retrospectively.According to the results of immunological analysis,the patients were divided into three groups: 9 cases treated with cyclosporin A(CsA);21 cases treated with vincristine(VCR);11 cases treated with high-dose dexamethasone(DXm).Curative effects were observed for 4 weeks consecutively.Results 6 cases(66.7%)treated with CsA,15 cases(71.4%) treated with VCR effectively and 7 cases(63.6%) treated with high-dose DXm were resulted effectively.Abnormal cellular immunity of the patients treated with CsA were improved,however,the level of PAIg in last two groups obviously reduced. immunity mechanism of refractory ITP;fairly satisfied effects might be obtained through taking direct treatments in view of the situation.Conclusion According to different immunity mechanism of refractory ITP,fairly satisfied effects might be obtained through taking direct treatments in view of the situation.
