Chronic urticaria associated with autoreactivity to progesterone

Autoimmune progesterone dermatitis is a rarely recognized cutaneous disorder revealed by recurrent and cyclic skin eruption with variable morphology, during the luteal phase. We report the case of a 42-year-old woman with chronic urticaria since the age of 38 with premenstrual exacerbation. Intradermal progesterone tests revealed a strong reactivity against this hormone. Treatment with thalidomide was effective but induced a peripheral neuropathy. Oophorectomy was performed with clearing of the cutaneous lesions.
