Drug Treatment of Drug-induced Liver Injury:Analysis for 262 Cases

OBJECTIVE:To investigate the onset regularity and therapeutic efficacy in the treatment of drug-induced liver damage.METHODS:The clinical data of 262 cases of drug-induced liver injury were retrospective analyzed.RESULTS:Of the total 262 cases,117 cases (44.6% ) were hepatocellular injury ( type A),32 cases (12.2% ) cholestasis (B),and 113 (43.1% ) mixed pattern (C).68.7% were induced by antineoplastic drugs followed by antibiotics (8.4% ),Chinese herbal medicines (6.5% ),antituberculosis drugs (4.5% ),antifungal drugs (4.6% ),and antiepileptic drug (3.8% ) etc.The common hepatoprotective drugs were reduced Glutathione (a) (152 cases/58.0% ),Tiopronin ( b) (85 cases/32.4% ),Polyene Phosphatidylcholine ( c) (65cases/ 24.8% ),Magnesium Isoglycyrrhizinate (d) (44 cases/16.8% ) and Ribonucleic Acid (e) (20 cases/7.6% ).The effective rates of drug ( b),( d) and ( e) were the highest while those of drug ( a) and ( c) were the lowest in treatment of the type A and type C liver injury.For drug (a) and (b),used either alone or in combination showed no significant differences in effective rate (P0.05).The effective rate of drug (c) was higher than that of drug (a) for B type liver injury.The combination of drug (c) with drug (a) was more effective than used singly for type A and C hepatic lesion.Higher cure rates were noted when drug (d) used alone for the treatment of type A liver injury or used in combination with drug (a) for the treatment of type A,B and C liver injury or with drug ( b) for the treatment of type C liver injury and when drug ( e) used alone for type A liver injury.CONCLUSION:Liver injury sometimes occurs in medical activities,and most of patients can be cured by liver protectants.Treatment of type A and type C liver injury with Glutathione or Tiopronin showed no significant differences in effective rate as compared with the combination of the two.Single use of Glutathione or Tiopronin is contributive to reduction of both medical costs and drug abuse-induced side effects.Polyene Phosphatidylcholine alone has satisfactory curative efficacy for type B liver injury.Used either alone or in combination with reduced glutathione or tiopronin,Magnesium Isoglycyrrhizinate has high cure rate for type A,B and C liver injury,and which remains to be studied further.
