Gender Disparities in ADHD: Impacts on Diagnosis, Treatment, and Long-Term Outcomes

摘要

Attention-deficit/hyperactivity disorder (ADHD) is a common neurodevelopmental disorder characterized by persistent patterns of inattention, hyperactivity, and impulsivity that interfere with daily functioning. While ADHD has historically been conceptualized as a disorder primarily affecting children, growing evidence demonstrates that it frequently persists into adulthood, with an estimated prevalence of approximately 4–5% among adults. For decades, ADHD has been disproportionately associated with males, largely due to the overt and disruptive nature of hyperactive and impulsive symptoms more commonly observed in boys. This historical bias has shaped both clinical recognition and diagnostic criteria, resulting in a framework that prioritizes externalizing behaviors. In contrast, females with ADHD are more likely to present with inattentive and internalized symptoms, including distractibility, disorganization, and difficulties with executive functioning. These symptoms are often subtle, less disruptive, and more easily attributed to personality traits or comorbid psychiatric conditions, leading to systematic under-recognition in females. Recent literature suggests that ADHD may be equally prevalent in women, yet remains significantly under-diagnosed and under-treated. It is estimated that millions of women worldwide are affected, many of whom are not identified until adolescence or adulthood . This delay in diagnosis is clinically significant, as untreated ADHD is associated with a wide range of adverse outcomes, including impaired academic performance, occupational instability, relationship difficulties, and increased risk of comorbid conditions such as anxiety, depression, and substance use disorders. In addition to differences in symptom presentation, biological and hormonal factors further complicate the clinical picture in women. Fluctuations in estrogen and progesterone have been shown to influence neurotransmitter systems involved in attention and behavior, contributing to variability in symptom severity and response to treatment across the lifespan. Despite these known differences, current diagnostic and treatment approaches often fail to adequately account for gender-specific factors, reinforcing disparities in care. The persistence of these disparities highlights a critical need to re-evaluate existing diagnostic frameworks and improve clinical awareness of how ADHD manifests differently in women. Significance and Motivation: Understanding gender-based differences in ADHD is essential for improving diagnostic accuracy, reducing delays in treatment, and promoting equitable, patient-centered care. As future healthcare providers, physician assistants are uniquely positioned to recognize these differences and advocate for more inclusive and effective approaches to ADHD management.
