Treatment of Severe Frontotemporal Cerebral Contusion and Laceration by Decompressive Craniectomy with Large Bone Flap

Objective To study the therapeutic effect of decompressive craniectomy with large bone flap on severe frontotemporal ce- rebral contussion and laceration.Methods 171 patients with severe frontotemporal cerebral contusion and laceration were studied retrospec- tively.Decompressive craniectomy with large bone flap was performed in 31 patients and decompressive craniectomy with routine bone flap was performed in 140 patients.Patients were followed up for 6 months.The effect was assessed by GOS.Results In the group of large bone flap.12 patients had a good recovery or a moderate disability,9 patients had a severe disability or a vegetative state and 10 patients died. In the group of routine bone flap,26 patients had a good recovery or moderate disability,45 patients had a severe disability or a vegetative state and 69 patients died.The difference of therapeutic effect between these two groups is significant.Conclusion The effect of decom- pressive craniectomy with large bone flap on severe frontotemporal cerebral contusion and laceration was better than that of decompressive craniectomy with routine bone flap.
