Early detection of cognitive impairment associated with schizophrenia and timely intervention are instrumental for improving quality of life for affected individuals. As researchers pursue new medications, determining how best to screen for cognitive impairment has become increasingly important. However, such screening remains insufficient or entirely absent in many mental health clinics across the United States. Although various cognitive assessment tools are available, clinical guidance on selecting appropriate tools for rapid screening in community mental health clinics is lacking. This article outlines the value of addressing cognitive health, discusses the challenges that affect the feasibility and acceptability of cognitive screening, and delineates its role in treatment planning. It also offers practical recommendations for the implementation of screening in busy clinical settings and lists various brief cognitive screening tools that may be feasible to use. Consideration is given to approaches to creating or tailoring cognitive health toolkits for these settings. Key considerations for effective and efficient cognitive screening are identified, including allocating resources, selecting screening tools, integrating screening tools into clinical workflows, developing patient engagement strategies, and establishing proper follow-up procedures. This guidance aims to help mental health programs in evaluating and adapting their practices to maintain an appropriate focus on cognitive health.
